Place:  Dell Children’s Medical Center

I.               Presentation by Dr. Sujit Iyer entitled “Visual diagnosis in the pediatric emergency department. (1905)

At first I though that this was going to be a presentation on vision.  Boy was I pleasantly surprised.  Dr. Iyer’s presentation was about making a diagnosis based on what you see.  His presentation consisted of some cool slides and pediatric case studies.  The following are just a few:

A.    Porencephalic cyst and growing skull fracture

In this case, a 6 month old had fallen 3 months prior.  Patient’s parents stated tha the bump was getting bigger.  The patient was diagnosis with a porencephalic cyst.  No child abuse charges were filed; the patient was neglected by not seeking medical care sooner.

B.   Staph scalded skin syndrome

§  Excusive disease of children

§  Classically starts in unrecognized sites (nose, throat, umbilicus).

§  Epidermolytic toxins

§  Classic signs:

·         Tender, red rash

·         Progress to bullae then exfoliation

·         Nikolsky sign – fragile skin area

·         Desquamation/perioral crusting

§  Treatment – fluids, fluids, fluids!  Treat like a burn victim

·         Antibiotic of choice is Clindamycin

·         Skin usually heals without scarring.

C.   Caustic Ingestion – Ingestion of hydrochloric acid

§  No GI decontamination or dilution

§  Endoscopy:  Symptomatic or significant ingestion

§  Steroids for circumferential 2nd degree esophageal burns

§  Treat the patient’s pain

D.   Airway Foreigh Bodies

·         Preschool/toddler age

·         Sudden episode choking/coughing

·         Complete vs partial

·         If H&P consistent with aspiration, proceed directly to rigid bronchoscopy.

·         FYI – peanuts and popcorn do not show up on xray.

E.   Disc Batteries

·         Double density or step off – this is not a coin but a battery.

·         Can cause esophageal burn and perforation within 4-9 hours

·         More prevalent in electronic devices

·         If above GE junction, patient needs to get scoped.

F.    Rare earth magnet ingestion

·         Very strong.  If 2 objects are in the bowel, they can find each other and cause a perforation.

·         Use a compass over body to see if the object is magnetic.

G.   Hematocolpos

·         Imperforated hymen

·         Abdominal mass

·         Primary amenorrhea

·         Treatment – Surgical

H.   Nerve Paralysis – Winged Scapula

o   Infection

o   Demyelinating

·         Parsonage-Turner Syndrome

o   Long thoracic nerve palsy
